• Payment is due at time of service. I accept cash, checks and credit cards.

• I accept HAS and FSA cards, same as cash.

• I can provide you with the necessary paperwork to seek out-of-network reimbursement from your insurance company.

• I offer sliding scale fees if necessary. Please ask me about this up front.
